 Dental Crowns and Fixed Bridges


Summary: In the field of dentistry, the crown and bridge refers to the restoration of your natural teeth that are damaged, decayed and missing. Crowns refer to a restoration of a single decayed tooth while a bridge is a fixed replacement of atleast one missing tooth permanently attached to one or several crowns. There are three main types of bridges, they are the traditional bridges or commonly known as fixed bridges, resin bonded bridges or also known as Maryland bonded bridges and cantilever bridges. Once treatment is completed, proper dental hygiene, including brushing and flossing, is necessary to maintain healthy, bacteria-free teeth, gums and your crown and bridge.

Sa larangan ng dentistry, ang "jacket crown" at "bridge" ay tumutukoy sa mga artipisyal na material na kapalit ng nabulok at nasirang bahagi ng ngipin o mismong kapalit ng isa o ilang buong ngipin na nawala o nabunot na.
Ang "jacket Crown" ay pamalit sa malalaking bulok o sira na kung hindi man ay buo ay sumasaklaw sa halos buong korona ng ngipin (Ang korona or "Crown" ay ang parte ng ngipin na nakalitaw sa ibabaw ng gilagid at ang parteng nakabaon sa gilagid at buto ay tinatawag na Ugat o "Roots"). Kung hindi sira ang buong korona, ang treatment na kinakailangan ay "pasta" lamang. Kaya tinawag na "jacket" ang treatment na ito ay sa dahilanang ang artipisyal na material ay bumabalot sa buong korona ng ngipin, samantalang ang pasta ay nakadikit at nakapatong o nakasuot lamang sa isa o ilang bahagi ng ngipin.




Ang "bridge" naman ay pinagdikit-dikit na jacket crown upang pamalit sa isa o mahigit na ngiping bungi o nabunot na. Ang mga bakante o bungi ay pinapalitan ng "false teeth" na nakadikit ng permanente sa katabing ngipin na may jacket. Kaya mukha itong "tulay" na may dalawa o higit na posteng ngipin sa magkabilang bahagi o gilid ng bungi. Ang pagkakaiba nito sa pustiso (removable dentures) ay hindi ito tinatanggal dahil permanente itong nakasemento sa ngipin, kaya sa Ingles ito ay tinatawag ding "fixed dentures".
Pagkatapos ma-eksamin ng dentista ang iyong mga sirang ngipin at mga bungi, at nasuri na rin ng mabuti ang iyong dental and medical history, siya ay handa ng magbigay ng kanyang opinyon kung anong klaseng treatment ang nababagay sa iyo. Hindi lahat ng malalaking sira ng ngipin ay pwedeng palitan ng Jacket. Minsan kapag sobrang laki na ang sira na halos wala ng paglalagyan ang jacket, may mga kinakailangan pang karagdagang treatments na gawin upang madagdagan ang laki o haba ng natitirang ngipin (upang magkaroon ng sapat na paglalagyan ng Jacket). Minsan naman ay wala na talagang pag-asa at kailangan na lang bunutin ito.
Ang jacket crowns at bridges ay gawa sa iba’t ibang materyales tulad ng plastic, porcelain; at metal katulad ng gold, stainless steel o kombinasyon ng ilan sa mga ito. Ang porcelain crowns ay isa sa may pinakanatural na anyo dahil sa lapit ng kulay nito sa totoong ngipin. Ang gold crowns naman ay maituturing na isa sa pinaka matibay. Sa kasalukuyan, marami pang mga makabagong materyales na ginagamit na masasabing may mas maganda pa sa ordinaryong porcelain at maitututring na singtibay ng metal ngunit ubod nga lamang ng mahal. Isa sa mga makabagong materyales na ito ay ang tinatawag na Zirconia.
May tatlong pangunahing desenyo naman ng mga bridges, ito ay ang mga sumusunod:
1. Traditional bridges: Ang false tooth o teeth na pamalit sa bungi ay nakakabit sa buong jacket crowns ng mga katabing ngipin sa magkabilang gilid ng bungi.
2. Resin bonded bridges na tinatawag ding Maryland bonded bridges na pangunahing ginagamit sa mga "front teeth". Ang pagkakaiba nito sa traditional bridges ay ang false teeth ay nakadikit o nakasemento sa likod lamang ng mga katabing ngipin. Sa desenyong ito, hindi kinakailangan bawasan ang buong korona ng ngipin hindi katulad ng sa traditional bridges. Maaaring hindi galawin ng dentista ang harap o mukha ng ngipin dahil sa likod lamang ang kinakailangang bawasan. Maituturing na mas mahuna ito dahil nga sa likod lamang ng ngipin nakakabit ang false tooth. Depende sa materyal, minsan ay mas mura ito kaysa sa traditional fixed bridges.
3. Cantilever bridges ay ginagamit naman kung hindi masyadong ginanagamit sa pag-nguya ang lugar na paglalagyan nito sa kadahilanang sa halip na sa magkabilang gilid ng bungi ang mga ngiping gagamiting pang poste, sa iisa lamang gilid ang pagkakabitan o pagdidikitan ng false tooth. Ang kabilang gilid nito ay hindi nakakapit sa katabing ngipin kaya ito ay rekomendado kung iisang ngipin lamang ang bungi.
Upang malagyan ng maayos na jacket crown, kinakailangang maihugis ng dentista ang natitirang ngipin na aakma sa nararapat na hugis ng jacket crown. Kung buhay pa at malusog ang nerve ng ngiping huhugisan, magdudulot ito ng pangingilo habang kinokorte ito ng dentista, ngunit huwag kang mag-alala sapagkat meron namang "pang-pamanhid" o "anesthesia" na ilalagay ng dentista para ikaw ay maging komportable habang ginagawa ang treatment. Kukuhanan ng isang "impression" o sukat ang iyong mga ngipin upang lumikha ng pinasadyang jacket crown na gawa sa isang commercial o professional dental laboratory. Sa kadalasan nangangailangan ang treatment na ito ng mahigit sa apat na bisita sa dentista sa loob ng dalawang linggo. Mahirap madaliin ang treatment na ito dahil napakahalaga ang tamang pagka-akma ng crowns sa paglalagyan nito. Ang kaunting pagkakamali sa sukat ng nagawang crowns at bridges ay magdudulot ng maliliit na "leaks" o "gaps" (puwang) na maaring daanan ng bacteria papasok sa loob ng ngipin. Ito ang pangunahing dahilan kung bakit nabubulok at namamaho ang mga ito.

Kapag ang proseso ay nakumpleto na, tamang dental hygiene, kasama na roon ang tamang pagsisipilyo at flossing, ay kinakailangan upang mapanatiling malusog at halos bacteria-free ang mga ngipin at gilagid lalo na yung may jacket crowns and bridges. Ang mga pamamaraang ito ay tumutulong sa pagpigil ng pagkakaroon ng sakit sa gilagid at pamumulok hindi lamang sa mga ngiping may jacket pati na rin sa lahat ng ngipin sa iyong bibig. Kung bibigyan ng tamang pag-aalaga, ang iyong jacket crownsat bridges ay tatagal ng maraming taon.
