as far as i know, not all malocclusion has a direct relation with TMD, to have a TMD problem, there must be a faulty positioning of the TMJ, but malocclusion isnt the only contributing factor...

in reality, extraction of 1st bicuspid if recommended as necessary would not directly cause a TMD...

for TMD problem to arise there must be a disruption, improper positioning, disfigured, malpositioning of TMJ, if upon extraction of 1st bicuspid, you create that problem then it would be a intentional TMD, however if upon and after the treatment, you are able to return the TMJ to its normal functioning position then i dont see any problem=)
